We have a newly developed quality assurance form to help keep us on track with several activities. This form is used with our internal annual quality assurance for programs/divisions. We have several new leadership team members since our last accreditation cycle, this document helps to remind them of some of the broad accreditation activities that apply to all of leadership. The idea really started with brainstorming a way to better capture evidence and data for activity 19.1, utilization data for underserved, at-risk and vulnerable populations identified in the CHA. We are still in the testing and refinement phase, but this is still a good starting point for us.
